Your Rights & Responsibilities as a Patient

  • As a patient you are entitled to

    • Receive high-quality, respectful, and non-discriminatory care.
    • Be informed about your health and participate in decisions.
    • Know who is providing your care.
    • Receive clear explanations about treatments and medications.
    • Request or refuse treatment as allowed by law.
    • Appoint someone to speak on your behalf if needed.
    • Access your medical records and receive copies upon request.
    • Have your pain assessed and managed promptly.
    • Access a translator or interpreter when available.
    • Request a change in provider if appropriate.
    • Wear personal or religious items unless they interfere with care.
    • Receive an estimate of charges and understand billing policies.
    • Report concerns or complaints without fear.

  • As a patient, we ask that you:

    • Provide accurate health and medical history.
    • Ask questions if you don’t understand your care plan.
    • Follow care instructions and treatment plans.
    • Arrive on time and notify us if you must cancel.
    • Respect staff, other patients, and facility policies.
    • Report changes in your condition or safety concerns.
    • Arrange for someone to accompany you home after surgery.
    • Be involved in your pain management and recovery process.
